Thousand Oaks High School Lancer Baseball Athletic Clearance

The TOHS Athletics Department requires student athletes to obtain clearance to participate in athletic activities each year. CVUSD utilizes the online service AthleticClearance.com for this purpose. Students can obtain clearance online by doing the following:

 

  • Create an account at AthleticClearance.com
  • There is a tutorial video on the home page to assist you.
  • Once you have created an account, click button "Start Clearance Here". Select the year, school, and sport for your athlete.
  • Click the "Submit" button at the bottom of each page to save your progress.
  • All forms require signatures which can be done as an electronic signature for both parent and athlete.
  • Print out Physical Packet (6 pages)-Obtain Phyusical or Have doctor fill out forms if Physical already completed
  • Upload Completed Documents from Physical
  • Upload a copy of the student's medical insurance card.
  • Once all forms have been completed online, the TOHS Athletics Office will review them.
  • You will be contacted if something is missing or needed.
  • You will receive a confirmation email when your athlete is cleared.

*No forms have to be brought to the Athletic Office.

Transportation

Transportation service (buses to away games) is not part of the TOHS Lancer Baseball Program. It is a service provided through CVUSD and should be paid when the student athlete makes a team and is due before the first away game. Please visit website above for all forms and information

 

  • Student Transportation permission slip
  • Personal Vehicle Use form

 

There are two ways you can make your payment. You can pay by credit card through the TOHS website Student Store Portal

https://thousandoaks.revtrak.net/, or you can pay by cash/check in the Athletics Office. Checks should be written payable to TOHS and should include the student's name on the check. Students who quit their team or are cut from a program for any reason are not entitled to a refund.

 

Fees are as follows: 1st Sport $124, 2nd Sport $71, 3rd Sport $55. Maximum per student $250, maximum per family $500.
